Registration

This is for NEW families who have never been enrolled in Chautauqua.  If you are a returning family who was previously enrolled in Chautauqua and then withdrew, please contact our Registrar (contact info below).

To register your child for Chautauqua Elementary School you will need to complete the form accessed via the button below. 

Be sure to select the correct school year! 

You will need to have digitial copies (photo ok) ready to upload of Birth Certificate, Proof of Residency, Immunizations and any legal documents (if applicable).
